At Cedar Crest, many of our patients come to us
directly after a hospital stay. If the attending physician determines
that a patient is ready for release but needs further care before
returning home, a case manager at the hospital is informed. The
case manager will then present the patient and his or her family
with a list of appropriate subacute facilities in the area. In other
cases, a family member who is concerned about a loved one's
health seeks out such care. A pre-screening then takes place to
determine whether skilled nursing is the appropriate level of care.
When a referral is made to Cedar Crest, an evaluation is conducted
to determine the patient's particular needs, treatment goals
and level of acuity. If we determine that we can care for the patient's
needs, a transfer to Cedar Crest is usually made within 24 hours.
One of our professional staff will immediately contact the patient's
physician to determine how patient progress reports and other communications
will take place.

Upon admission, a personal care plan will be developed
by our interdisciplinary team along with a guide as to how the care
will be implemented. This plan will be monitored and modified as
necessary throughout the patient stay. Family and caregivers' involvement
in this process is highly encouraged and recommended.
To assure that each patient is fully informed of costs before admission,
methods of payment will be discussed during the pre-admission evaluation
at the hospital. Payment may be made by most major insurance plans
that participate with this level of care and assist with payment
when all criteria for admission are met. Cedar Crest is approved
by several managed care plans.
