Each person admitted to Cedar Crest Subacute & Rehabilitation Centre receives a clinical evaluation where any existing medical, physical, cognitive or psychosocial problems are identified. Under the direction of the patient’s physician and our Nurse Practitioner, The Cedar Crest Team designs a tailor-made treatment program. We firmly believe that the individuals in our care and their families are the most important members of the team. We encourage them to participate in the creation and implementation of the overall treatment plan.

Cedar Crest is unique among other long-term care facilities by our employment of full-time, in-house Nurse Practitioners who support the medical team. Our Nurse Practitioners can assess ill patients, conduct histories and physicals, review laboratory results, respond to emergency situations, diagnose, prescribe and treat acute and chronic conditions.

Cedar Crest has a competitive edge with the tremendous advantage of an in-house, full-time team of licensed therapists who offer physical, occupational and speech therapy six-days per week. This means patients and staff get to know each other and patients receive exactly the therapy they need to maximize outcomes.

Cedar Crest staff receives ongoing training and education, and are actively involved in positive change through Quality Improvement committees that review issues and recommend solutions in areas such as fall prevention, pressure ulcer prevention, and pain management.
