The Importance of Medication Management #ElderCareChat

Linda Cooney, our Director of Home Health Services participated in a Twitter discussion this past Wednesday regarding medication management. There were over 30 participants in the #ElderCareChat and Linda was able to provide valuable insight on medication management.

One of the advantages of The Cedars home health program is that we work with patients and their families in the area of medication management. We are able to educate and teach not only the patient but the family members while working with physicians and pharmacists to make sure that patients are taking medications effectively and safely. Our biggest responsibility is trying to prevent our patients from having another hospitalization.
