To find home care services for yourself, a family member or a patient, please contact us by phone, fax or email. We are happy to discuss eligibility or payment for home care needs and will work diligently to streamline the admissions process. Once the initial referral information is received, a registered nurse from Home Health at The Cedars will make a personal visit to the client’s residence to discuss medical care requirements, answer questions and to create an individualized plan of care. Cedar Home Health accepts payment from many sources including:

• Blue Cross and Blue Shield

• United Healthcare

• Tufts

• Medicare

• Medicaid

• Long Term Care Insurance

• Short and Long Term Disability Insurance

• Private-Pay – We accept payments by check or credit card (MasterCard and Visa)

To qualify for insured home health care services, the following criteria must be met:

• Have orders from a physician concerning a treatment plan

• Have a treatment plan that is reasonable and necessary under the qualifications of coverage provided by the individual’s insurance

• Be Homebound
