How the Cedars Distinctive Nursing Program Works

At The Cedars, our nurses are dedicated to the highest level of patient care.

When it comes to finding a rehabilitation facility, or care provider for a loved one, you want to ensure that the facility you choose is reputable and qualified, able to provide your loved one with the care that they need.

At The Cedars, our commitment to providing patients and residents with the highest level of care is behind everything that we do.

Our commitment to our patients is one of the driving factors behind our distinctive nursing program.

How the Distinctive Nursing Program Works

When it comes to long-term care services in Rhode Island, short-term rehabilitation, or in-home care, our nursing program involves using a Primary Nursing model. This means that specific Registered Nurses and nursing assistants are assigned to each patient for the duration of their stay.

We believe that this relationship-based approach is beneficial for the patient’s well-being. This approach allows us to provide individualized care and a personalized plan for each patient.

Each of our nurses is highly skilled and trained, and are qualified to accommodate any health-related issues that you, or your loved one may have.

Whether you’re looking for a short-term stay or a long-term care provider for your loved one, here’s what you can expect from the Cedars nursing program.

  • A Team Approach

For at-home care, our nursing program involves a coordinated team effort that is designed to ensure optimal patient care. Skilled nursing is provided by an RN or LPN who communicates frequently with the physician to update the plan of care.

  • Qualified and Caring Staff

Much of our nursing staff has been with us their entire career, choosing to utilize their nursing skills in our care center. Our nurses treat our patients with the utmost care, and have a strong focus on pain management.

  • Continuity of Care

The patient’s primary nurse follows them from the point of admission to discharge back to community. Our care focuses on everything from pain management, wound care, medication control, and on-going medical assessments.

  • Tools for High Quality Medical Management

Our tools offer high quality medical management including computerized bedside assessment, computers for lab and pharmacy reference, and wall-mounted units to allow nursing assistants to input patient vital signs and instantly access patient information.

Benefits of Patient Centered Care

Some of the benefits of providing patient centered care include:

  • Getting to knowing the patient as an individual

  • Responsiveness

  • Meaningful Care

  • Respecting the individual, their preference, and choices

The strong focus and drive behind our program is to provide the best care for our patients. We also offer patient and family education of disease processes and medication management.

Between the hospital and home, we provide person-centered care to over 1200 people each year. Our commitment to each of our patient’s and resident’s best interests remains the center of our focus.
